A Home Medicines Review (HMR) is when an accredited pharmacist checks the medicines you are taking at home.

 

Taking lots of different medicines can be complicated. Not taking them properly or mixing the wrong ones can have serious consequences.

 

A Home Medicines Review makes sure that your medicines are safe, you are taking your medicines correctly and your medicines are working for you.

Your GP will assess your need for a Home Medicines Review. 

What Happens During a Home Medicine Review?

Your GP will create a referral for a Home Medicine Review through HiQ.

 A trusted pharmacist in your local area will organise a time with you to review your medications at your home.

Your GP will share the recommendations with you at a follow up GP appointment.

What are the benefits?

It's a good idea to ask your GP for a home medicine review if you:

1. Are taking more than 5 medications

2. Started a new medicine
3. Have recently been in hospital
4. Feel unwell after taking medications
5. Have medicines prescribed by different doctors
6. Don't always remember to take your medicines
7. Are concerned about your medicines

How much does it cost?

You do not have to pay for a home medicines review. However, you might have to pay to see your doctor for the referral, unless they bulk bill.
