Referral Letter Templates

Domiciliary Medication Management Review (DMMR) (Item 900), also known as Home Medicines Review, referral letter templates for use in your practice management system are a quick way for GPs to gather the required information to share with the accredited pharmacist to complete the home medicines review.

GPs can share these templates with HiQ Health via HealthLink or upload to the HiQ Health platform so that HiQ Health can share this information with the identified accredited pharmacist.

 

Please use a generic DMMR template available in your practice management system with HiQ Health Pty Ltd listed as the community pharmacy.
